Meet Tao Xia, Founding System Architect at actAVA.ai
Tao Xia is a veteran systems architect with over 20 years of experience specializing in designing and scaling massive, distributed backend infrastructure. Currently the Lead Backend Engineer at actAVA.ai, Tao is at the forefront of AI orchestration, developing mission-critical systems that power healthcare AI workflows. His deep technical pedigree is anchored by a career of high-impact leadership, including scaling DoorDash’s consumer platform from 50,000 to over 2 million daily orders and establishing the foundational architecture as a Founding Engineer at Kindred. From architecting global data ingestion systems at Yahoo to managing cross-functional engineering teams, Tao combines a mastery of high-performance systems with the strategic vision required to build and sustain modern, AI-driven enterprises.
We took this Friday to interview Tao on the concept of Scaling Trust & Safety in AI Infrastructure.
Q: Many people focus on the safety of the AI models themselves, rightly so. Why do you believe safety must also be solved at the infrastructure level?
AI safety isn’t just about what a model generates—it’s about how the entire system behaves in production. In healthcare, AI outputs move through complex pipelines: data ingestion, orchestration layers, and integrations with downstream clinical systems. If your safety controls exist only within the model, you’re missing most of the operational risk surface.
Infrastructure is where we enforce guardrails consistently. By designing "Safe by Design" architecture, we ensure that AI systems behave predictably even as models evolve. To me, safe AI infrastructure looks like a well-designed distributed system: it requires observability, failure isolation, and deterministic control layers that govern non-deterministic models.
Q: You often say actAVA’s platform, KORA, is “Safe by Design.” What does that look like in practice?
It means guardrails aren't a layer we bolt on at the end—they are baked into how the platform reasons and acts. Every agent on KORA operates within defined boundaries. It knows what it’s allowed to do, what requires human review, and when to "fail safe" rather than just failing fast.
We achieve this through Structured Orchestration. AI shouldn't operate as a black box; every step in a workflow must be explicit and traceable. When an agent takes a path or hits a condition, that surfaces in real-time. This isn’t a "nice-to-have"; in healthcare, missed escalations affect lives. We built the system so that when a compliance team asks, "Why did the system do this?", we can provide the answer immediately.
Q: Healthcare executives are often skeptical of AI speed. How do you manage the tradeoff between moving fast and staying secure?
In healthcare, speed without safety isn't a feature—it's a liability. We don't treat them as a tradeoff. Instead, our architecture uses the safety layer as the foundation that makes speed possible.
Because our compliance infrastructure and "human-in-the-loop" checkpoints are already integrated, our customers can configure and deploy new agents quickly without reinventing the security wheel. We’re also serious about pressure-testing; we try to break the system before a customer ever sees it.
Q: If a partner asked you to prove the system is reliable and secure, what would you show them?
I’d show them the "receipts." We start with the fundamentals: our HIPAA compliance and SOC 2 Type 1 & 2 certification. These aren't vanity badges; they represent real, third-party-verified controls that are open for all to see on our actAVA Trust Center.
Then, I’d walk them through a live agent workflow. I’d show them the logs, the escalation paths, and the exact points where a human intervenes. In the world of high-performance backend engineering, claims that can’t be demonstrated are just marketing. We focus on transparency and end-to-end traceability because that is the only way to build lasting trust in the medical and life sciences space.
